We try to update it, but some users take themselves off the teams by unsubscribing to get the colors they want. If you wan accurate teams, just go into the team sheets.

Also, some people aren't added because they already have a primary color, and since the only purpose for adding people to groups is to give their name color, it's not necessary for the teams to be entirely accurate.
And as Lugia said, members take it upon themselves to add themselves or take themselves off.
